What is Home Health Care?

Home Health Care is an assistance service involving a full range of medical and custodial care. Care may include nursing care, therapy, and assistance with personal care. Physicians may order homecare after hospitalization or in lieu of hospitalization when skilled care may be accomplished safely in the home. Grace Home Health, in collaboration with the patient’s Physician, will develop a specific Plan of Care tailor-made to meet the individual needs of the patient. This Plan of Care will be monitored regularly and any changes in condition will be notified immediately to your Physician.

Admission Criteria

Admission to GHH is based on client’s needs and the type of services required which this company can provide. In order to make a determination about suitability for home health care services, it is important to provide relevant information during the admissions process thus, your cooperation is imperative. Your admission will be in addition to the Director of Nursing.

GHH does not discriminate on the basis of race, color, national origin, sex, religion, disability, or age in regards to client admission, client access to treatment, or to employment issues.

Our Agency has a policy to admit only those individuals for whom we can reasonably meet demonstrated needs in care and service. On an ongoing basis, our health care practitioners monitor patient/client needs. When a situation may occur, in which a patient/client can no longer be accommodated, ether by GHH personal or through contracted service provided by other agencies, we will develop a discharge or transfer plan for the patient/client.

Safety and security of our patients/clients and our staff are important considerations when reviewing admission of a new client/patient. We reserve the right to withdraw service without prior notification to either the patient/client or referral source.
