Home Health Care is an assistance service involving a full range
of medical and custodial care that may include the following:

- Companion/Sitter Services
Keeping in mind that every individual is unique and might have
needs that are different from others, Grace Home Health, in collaboration
with the patient’s Physician, will develop a specific Plan
of Care tailor-made to meet the individual needs of the patient.
All our services are provided by qualified healthcare professionals
who are available 24 hours a day, 7 days a week to assist you with
your specialized home care needs.
